How to Submit

Your Medical Records

You can submit the following medical records so we may verify your qualifying condition:

  • Chart notes from a doctor, physical therapist, chiropractor, or other licensed medical professional that show the condition diagnosis.

  • Military paperwork that lists a qualifying condition

  • Disability paperwork that shows a qualifying condition

  • A note from your doctor that lists your qualifying condition

  • A screenshot of your diagnosis list from MyChart or other Patient Portal Systems

  • Aftercare summaries from your doctor that list the condition

  • Medical imaging that shows your condition clearly (we are not radiologists, so if it isn’t obvious from the image, such as hardware or an obvious injury, then we can’t use it). You can also submit the report from the radiologist that describes the condition.

  • Not Acceptable: We cannot accept your own health summary written by you. It must be written and documented by your health care provider so we may fulfill our legal obligations to verify that you have been diagnosed and had other forms of treatment besides medical cannabis.

  • If you do not have medical records: We can still help. Please sign up for the Initial Medical Marijuana Authorization, AND the Add-on to Establish Care for a Qualifying Condition.
