How do I book an appointment?

Do I need medical records to qualify?
The doctor will need to verify your qualifying condition with official medical records from your healthcare provider. For types of records we accept, see this list
What if I don’t have medical records or can’t access them?
You can select the add-on to establish care for a qualifying condition with our doctor during an in-person appointment. The doctor will evaluate your symptoms and discuss treatment options, and create the proper documentation.
Do I have to come to the office in person?
Only one time, for the first time authorization, to fulfill WA state law. If you are a renewal, you can schedule by video consultation. If you were not seen by our doctor for your last authorization, we will need a copy of the previous authorization or recognition card to determine your eligibility to renew through telemedicine.
What do I need to qualify?
We will need a Government ID showing a Washington address, medical records showing a qualifying condition, and an appointment of about 15 minutes.
What if I live in Washington but my ID is from out of state?
Please bring a copy of a lease or utility bill in your own name showing your Washington State address.
What age qualifies? What if I am under 21, or a minor?
Any patient over age 18 may schedule an appointment on their own. This is the only way you can legally purchase cannabis between ages 18-21. Patients under age 18 must be accompanied by a parent or guardian who will be their designated provider to purchase cannabis. Patients under 18 must renew their authorization every 6 months.
Is getting an authorization private? What agencies have access?
Getting a medical authorization is completely private and protected by HIPAA privacy laws. We do not share your authorization with anybody, nor do we submit them to any agencies. Only you can share your authorization, for example, to register in the WA State database.
