The Process:
1. Confirm that you have a qualifying condition that has been diagnosed by a physician, chiropractor, or other medical provider.
2. Upload medical documentation of your qualifying condition.
**If you don’t have medical records, we can still help — you can set up an appointment to have your symptoms evaluated and diagnosed by our physician, and once your medical diagnosis is established, then we can continue with your authorization.
3. Make a 15 minute in-person appointment for a physical exam with our licensed physicians.
4. Receive your authorization the same day as your physical exam is completed, with recommendations for treatment of your condition. It's that easy!

Qualifying Conditions:
Chronic Pain
- Ex: Joint pain, back pain, migraines, chronic headaches, abdominal pain, post-surgical pain, fibromyalgia, phantom limb pain, etc 
Nausea or Vomiting
- Ex. Chronic nausea, motion sickness, nausea due to medications or chemotherapy, nausea related to gastrointestinal condition, effects of anxiety or panic 
Appetite Loss
- Ex. Loss of appetite due to chronic condition, medications, treatment of other disease, neurological disease, effects of anxiety or panic 
Cramping
- Ex. Muscle cramps, leg cramps, menstrual cramps, abdominal cramps. 
Muscle Spasms
- Ex. Musculoskeletal conditions, Muscular Dystrophy, ALS, myasthenia gravis, Parkinsons, dystonias, effects of kidney failure 
Post Traumatic Stress Disorder (PTSD)
- Ex. documented Acute or Chronic PTSD from traumatic events 
Spasticity Disorder
- Ex. IBS, Constipation, Restless Legs Syndrome, effects of stroke 
Epilepsy/Seizures
- Ex. Epilepsy, seizures caused by brain injury, infections, or medications

All patients must be seen for an in-person physical exam on the first visit. Renewals may be eligible for telemedicine appointments.
Authorizations are written for 12 months, except for minors under age 18, which are available for 6 months and require a parent or guardian.
Parking:
Free 2-hour street parking is available, along with several paid parking lots nearby.
Accessibility:
Appointment rooms are on the ground floor, and there are no stairs into the building. Bathrooms are available, but not ADA compliant. If you require parking very close to the building due to disability, there are typically spaces available on Saturdays next to our building.
